command button in toolbar

I took a look at http://www.erlandsendata.no/english/index.php?d=endownloadcommandbars
and im a bit stumped. What i want to do is take a macro that i saved as an add-in and create a button to activate the macro in the toolbar such that when you have the add-in active the toolbar button is available for the macro. Can someone point me in the right direction?
