Turning google doc single row multiple choice responses into multiple row responses

Hi,

I have a Google Doc with one multiple choice question. The rest of the questions are identifying information. We need to convert the single row responses into multiple rows. So each "Club Listing" should have a separate row, but the other cell info should be repeated for each.

I have been searching on this site, but cannot seem to find my answer no many how many options I have tried. The issue is that some have chosen one club and some more than one (top is 5).

Does anyone know how to handle this?

Three semi-colons will hide the value in the cell. Although most people use white font instead.
