Power Query from a web site having multiple parameters

I am trying to build a excel web query from a site that has multiple parameters. I tried few options but nothing works. Can someone guide me to how to build a web query from a website that has multiple parameters to select?


where IndexCode, and dates are parameters.

Any help will be appreciated.
