Look up phone number by criteria

Ok so I have an excel spreadsheet setup.
A B C D
State DEC Name Phone Number


What I am trying to do is search for a number under column B and then return the Phone Number in column D that corresponds on the same row.

=INDEX($B$2:$B$322, MATCH(E2,$D$2:$D$322,0), MATCH($F$2,$D$2:$D$322,0))


This does not work.
 

Excel Facts

Whats the difference between CONCAT and CONCATENATE?
The newer CONCAT function can reference a range of cells. =CONCATENATE(A1,A2,A3,A4,A5) becomes =CONCAT(A1:A5)
In that case Excel cannot find the value.
In an unused cell use
=E2=B5
changing the B5 to look at the cell that should match E2, what does the formula return?
