Individual incident data

I have a spreadsheet that has numerous entries by individual. The sheet logs all accidents that have taken place on site and I want to be able to provide a summary by individual. I have tried pivot tables but that just shows how many times each individual has an entry. Ideally I would like to show the last 3 incidents by individual and then the brief description of what happened. So I think I need to show the name, date and then text. Is this possible?

Thanks for this, however I don't know where to start with this! I want to select the name in the results tab and have their individual record appear. Do I need to run this in the results tab, and if so how?

Excel Facts

Can you AutoAverage in Excel?
There is a drop-down next to the AutoSum symbol. Open the drop-down to choose AVERAGE, COUNT, MAX, or MIN
When you started this question you gave no real plan.
I gave a suggestion and you said OK but lets search for name and last 365 days.

I said we would copy to sheet named Results.

So I'm not sure why now your saying

I want to select the name in the results tab and have their individual record appear

What do you mean select the name in the results Tab.

I was going to copy the rows to the results Tab.

Are you now saying you have never used a Vba script.

I told you a Inputbox would pop up on the screen asking you to enter a name.

So are you saying you did not run the script?
