I need a formula that will give me the following results in column D. If there is multiple cells with in column A and column B and C show active for any of the repetative cells or non repetative cells then the results should be "OK" otherwise the results should be "Violation."

Do the Blanks in both also require multiple records for that item as the others?
It may be a good idea to summarize your requirements in an updated all encompassing requirement list.

Yes, the blanks in both column B and C also require multiple records same as the other. Below is a summary.
