I'm trying to create some easier forms for a health questionnaire. I'm not having much luck.

=IF(G4=Male,AND(G3>=45,"1",""),IF(G4=Female,AND(G3>=55,"1","")

What am I missing? Or should I use the look up formula?

Excel Facts

Copy a format multiple times
Select a formatted range. Double-click the Format Painter (left side of Home tab). You can paste formatting multiple times. Esc to stop
I'm guessing you mean:

=IF(OR(AND(G4="Male",G3>=45),AND(G4="Female",G3>=55)),"1","")

Note this will return a text value "1", not a numeric 1.
