formula to transfer from one sheet automatically to the second sheet?

I am having problems trying to get a name from the first sheet (Child Data) to automatically transfer to Sheet 3 (child A) Is this possible if so could someone please help! :)

Second Problem is on sheet 1 (child Data) i have he child's DOB is it possible for excel to use this date and turn it into months on sheet 3 (child A) between certain date fields? for example [TABLE="width: 259"]
