Formula to calculate repetitive cells with matching information in multiple columns

I need a formula that will give me the following results in column D.
If there are multiple cells within column A and column B and C show active or they are both empty for any of the repetitive cells or non-repetitive cells then the results should be "OK" otherwise, the results should be "Violation."
