Creating a form from a query problem!

Hi guys,

I am trying to create a form from a query. I have done it before but I seem to be having a problem setting it up again. I have various tables in my query with various relationships. I setup up my form and add the fields in design view but when I go out of design view I just get agrey sheet with no fields. They are present on desing view but not in form view. The query is a select query and it was a select query that I have used before. What am I doing different?

Please help me!!!
