Adding comments through pop up

I need to add a kind of advanced comments to cell in a worksheet. Say if I click in cell A2 and press a button, a pop up appears that allows me to add comments.

The pop needs three values, 1. A drop down menu of two options (D1, D2), 2. Text comment, 3. Text comment.

Afterwards, when the mouse moves over over the cell (A2), the comments show up like regular comment with three lines corresponding to the three comments entered through the forum.

This should work on any cell in the worksheet.

If this is possible, I will appreciate all the help I can get.
