We have a macro written like 20 years ago that now is not allowed to open anymore from Sharepoint because Defender is blocking the file and it just says the error message below. Macro's included are quite complex and spread into multiple modules. Is there a best practice on how to find the culprit and resolve this without removing all the macros?

Is the file really XLSM? Written 20 years ago. AFAI remember XLSM came in with Office 2007.
If it is a XLS file use the correct extension. This is the meaning of the Error message. And check the File Block settings in Trust Center.
I don't think Defender has anything to do with the problem.
