W-2 FORM IN EXCEL WORKSHEET

I've been trying to find a W-2 Form in excel, but haven't been able to locate one. Does anyone out there have one? I came across a W-2 Form on the Microsoft Office Online Templates, but it is in Excel 2007 and I have Excel 2003. The web site gives a way to download the template, but I was unable to extract it. Can anyone help???
