Vlookup with Multiple Results in same cell?

I am trying to take data input like this:
input.jpg


With a formula to output it like this:
output.jpg


Is this possible??

THANKS! This would save hours of work every week.
 

Excel Facts

VLOOKUP to Left?
Use =VLOOKUP(A2,CHOOSE({1,2},$Z$1:$Z$99,$Y$1:$Y$99),2,False) to lookup Y values to left of Z values.
The count is in Column W, isnt it?

Yes that is correct, Column W. (It is actually in column W on another sheet than the one I am putting the output formula on, but I got the formula to work by just changing your reference to the other sheet in the formula)

Try this formula
=SUBSTITUTE(ACONCAT(IF($U$2:$U$9=A2,IF($W$2:$W$9<>"","; "&$V$2:$V$9&"("&$W$2:$W$9&")",""),"")),"; ","",1)

Ctrl+Shift+Enter
