VBA Code to prepare a survey

I want to create a checklist in excel and I have 5 Questions.

If any one of the question answer as "Yes" immediately it shall generate a email to given address and close the excel sheet.

if the answer is "No" of the 5 questions it shall be closed at the end of the questions.

Anybody can support on this.

dont make it harder for you, use your google account and go to Google forms, create a survey, set-up email and extract to CSV. done no macros.

actually it is not survey i need it in excel with combo box
