Vba code to data map many to one

Hi excel users, I am facing an issue regarding data mapping for many-to-one data. Let me give you an overview of the idea I am facing.

i have attached a screenshot on the data entry software i am planning to build. i have planned to map the blank rows to a table in the same workbook.

any idea how do i send the data with one code which has multiple rows of additonal info to a table wherein it stores all the data.

any help is highly appreciated
