Transfer data from one sheet to another, as well as complete "form"

Hi,
I need some help!!!
I have a workbook for when my staff make errors. My admin person will complete the log by filling in data from paper logs.

What I have so far is a primitive spreadsheet where you put the number of errors in a particular cell, against an operator name.

I have copied a bit of macro where, if data is placed in cell d,k,r,y or af, a form pops up and asks you to enter "name", "number of Errors", "Date" & "Comments". This data is then transfered to another sheet (errordata). This all works fine!!!

What I would like to happen is for a macro to automatically run when cell d,k,r,y or af is edited and copy cell A ("name") and the cell which has the data which was just added - either d,k,r,y or af ("number of errors"), and then open the form for the admin person to complete the date and comments, which would then all be put in sheet "errordata"

Can anyone assist???

I have an example sheet which I could attach if someone could tell me how to.
