Take User form data and put in the body of outlook email

Hey guys I'm still very very new to this, I've been working in excel for a month total now, if that. I have a userform that auto pops the date, and user login, has a combobox to pick options from, as well as a notes field. I was able to write/mod/copy enough so when you click on the 'submit' button it will open a new email in outlook but I can't get any of the data put into the userform into the body of the email can someone please help me??
 
Can you post the code you currently have that opens a new email? Also, what are some of the controls (options and notes field) names to help you out.
