Switching from paper card stock to Excel

At work we have a paper card stock "form" and a paper log book used as a control whenever a system is opened. Every time someone needs one of these forms we hand fill out one of these cards and make an entry into the log book (well it's time to move into the 21st century). The log book part is easy, but I need to "Print" to form. Is the anyway that I can scan one of the cardstock form and import in into Excel so when I fill out the log book it will print one of the forms (pre-filled with data from the log book tab) Thanks
