Survey Response - Listing responders, not counting number of responses

Hello,

Long time lurker, first time poster here.

I'm trying to create a data management system and having trouble with this one last forumla – which is the difference between me being able to sleep at night and staying up ripping my hair out.


The below screenshot is a crude replication of what I want to do.

On the front end, the respondents will give their responses, but on the back end I want to see who gave what response and add that data to a separate table.

If anybody can help I would be extremely grateful.

Excel Facts

Control Word Wrap
Press Alt+Enter to move to a new row in a cell. Lets you control where the words wrap.
Assuming there are only 3 possible responses and the headings are already in F2:H2, then :
