substitution in string, Ref another cell

LETS SAY I HAVE AN ADDRESS:
http://www.google.com/search?hl=en&c...=f&NAME+#%#2ADDRESSoq=&aqi=g10

Their are two words in their NAME and ADDRESS, I want use a formal or whatever to replace those words from another cell / column


so if I have this GOOGLE ADDRESS in one cell and two column NAME and ADDRESS with data in them, they will replace the words inside the search
