Hello Friends,

I need help in getting my checklist in shape. I would just want this checklist that has 2 radio buttons for each line item and that would have a formula to calculate all the Yes and No. How can I upload my working file here?

The one that I have right now does not seem to work as I expected lol I can't do Yes and No selection for each line item instead I can only select 1 from everything. Thanks in advance!

How can something be yes and no at the same time?
Why do we see 3 buttons when you say there are only 2?
To answer your question, you can't upload files here. You can only provide a link to a file sharing site where you've placed a copy of your file and allow access to anyone with the link. Some people won't download from file shares and some will.

Sorry let me be clear. The requirement is for a use of this checklist to be able to select Yes OR No OR N/A in each of the line item. How can I make that happen? thanks for the info RE: Uploading of file here.

When in Design View you will have to make a group for each row then for each group (after all are grouped or as you go), select the 1st option. In the properties sheet, change the default group name, which usually ends up being the sheet name. Right now you can only select 1 for the whole sheet because all of your buttons are grouped by sheet. Looks like you got your work cut out for you.
