return value on match of list?

I've tried a number of options, but can's seem to get there - but I know this team will find it! (and sorry, cannot install xl2bb)
Sheet 1 - has list of last names, Group and Year

Sheet 2 - has list of last names - some match, some don't. i want the grp and level to move to sheet 2 and return false (or user defined) when a name is not on sheet 1.
