Rearranging data

Hi guys!!!!

I have this data of weight meassurements, i have one measurement for every hour of the last 5 years, but the format of the table its not useful for me, I need to change it to another format.
The one in green is the one i have, the one in yellow is the one I need.

[TD]I used the Concatenate funtion to change the date, but for the hours i tried using Transpose but it didnt work. I want all the data in 3 columns to make it easier for me, I would do it manually but it would take too much time, thats why I wanted to search for other opinion (a more experienced one)[/TD]

Ill be grateful for any help or suggestions
