Questions about helpfiles

When I went to Microsoft's msgbox documentation, I saw arguments for a help file and a context for the help file. I also saw that the extension is .HLP.

I have done some googling, but the information for the topic is quite sparse, even from Microsoft. These are the questions I still have after googling.
  1. Is .mshc the most modern extension?
  2. How do you ACTUALLY create a help file? (using which programs? Is there a manual for it?)
  3. In what ways can you use it in Microsoft
  4. And last but the weirdest: can you make a help file just within the base tools of Microsoft (excel, word, notepad, PowerPoint)

Pressing F4 adds dollar signs when editing a formula. When not editing, F4 repeats last command.
It has been a long time since I had to create help files. What little I can remember is, yes, basic word editors could create help files, but you'd practically need to learn another language to be able to write your own. We used Help Scribble instead because it was ridiculously priced for what you got ($99 US and eternal upgrades), relatively easy to use and it could create different file formats. It seems it is still around:
