Does anyone have a template for an Access (2003/2007) Questionnaire?

Hopefully that has different headings and when headings are changed the corresponding questions show? Answers as simple as Yes, No, N/A

i'm trying to design one (6 Headers, 8 questions each max) but my very limited Access knowledge is proving very difficult

The Excel Web App has a Survey builder that you can share online, and the results are fed back to an Excel workbook for analysis.

It's really easy to set up and use, and might make more sense than Access.
