Query or Filter?

I am working with data that contains multiple names that for each record. I have created a form with combo boxes that allow me to choose the name of individuals associated with records. I want to be able to filter or query records by the names that I choose from these combo boxes. I have tried both methods of query and filtering and cannot get it to work properly.

For the Query:
When I run a select query I can get one of the combo boxes to work, but when I try and tie the query to two combo boxes it does not work. I think this is because I don't know how to program the logic. I need the user to be able to choose a name from combo box 1 through combo box 5 or just from combo box 3. Can anyone help?

When I try to filter:
I am also able to tie one combo box to one field and display the records accordingly. If I try and tie to fields to two combo boxes it does not work.

Like i mentioned above, I don't want to have to have a button for every time I want to filter or query something. I must be missing something obvious.

Ctrl+S is used for Save. Ctrl+5 is used for Strikethrough. Why Ctrl+5? When you use hashmarks to count |||| is 4, strike through to mean 5.
