Problems with 64-bit Excel

I use 32-bit Excel 2010 and someone I'm working with recently converted to some version of 64-bit Excel. The original workbook that I wrote for him had a treeview in a userform and as you may know, the built-in treeview is no longer available in 64-bit Excel, so I rewrote the code in the userform to use a 3rd-party treeview that does work in 64-bit. He and I experienced a lot of problems and error messages caused by my not being able to test in 64-bit. We finally got to the point where there are no error messages, but for some unknown reason the userform doesn't show up and I'm stumped.

Can someone who has 64-bit click the 'Show Search Form' button and tell me if the userform shows up? If it doesn't please tell or show me what I need to do to correct the code, and if it does show up please enter some value like panel in the Key Word box and then click Search and tell me if you get some results.
Here's a link to the workbook.

Links? Where??
If Excel says you have links but you can't find them, go to Formulas, Name Manager. Look for old links to dead workbooks & delete.
With Office 365 Business - 64 bit, that part seems to work fine:
