populating a table using a form

I have a table that is such as the following:

Unit Category W1 W2 W3 ect...thru W52
unit one : 12345 sales 125
unit one : 12345 product 39
unit one : 12345 costs 300
unit two : 24569 sales 352
unit two : 24569 product 52
unit two : 24569 costs 659

in a form, the first text box requests the W#
the second text box asks for the Unit #
in the 3rd box, I want it to pull the corresponding Unit Name : Unit Number (as these fields are not seperate in the cell), every corresponding category for that unit (sales, product, cost) and the Week column that they need to do data entry so they just type it in and it goes into the table.

I would like the entry form to look like a datasheet as above only it will show one appropriate week at a time.

I have been trying and trying to figure this out with no luck. Could anyone help?
