populating a form, saving file and looping using a Macro

A supplier requires me to send them in an individual form(excel workbook) for each of my customers in order to receive a rebate on my sales. I have added a list of data on a separate worksheet to the "rebate workbook". The data columns are Customer Names, Account Numbers, Sales Volume and the discount amount I've requested. I would like to auto populate the customers name into the suppliers form, Save the workbook as the customers name, populate the form again with the next customer from the data set, save and continue looping 200 times.
