Populate selections of Radio buttons in separate sheet

Hi,

i am designing a survey question template in excel for which i have to use Option Button as part of form control.

in the survey there are questions with multiple answers (for each question user has to select any of the 4 options available.)

upon selecting any option for the question the result is populated in a separate sheet.
Eg. In Sheet A user for Q1 select Age group 20-30 from the Options 10-20, 20-30, 30-40, and 41 and above.
Resultant in Sheet B, Column A contains the question and Column B contains the selected option from Sheet A.

Kindly assist.

I was not aware that a option Button can be linked to a form Control

And I have never really used the Form Control

Or are you referring to a Userform
Or are you referring to a Form controls Option button


Please give more details like both sheet names
