Pop Up Message on Open

Hiya - I need help with some code.

I've created a form and need instructions in a pop up window to go with it.

What I would like to do...when you click the form to open - the first thing you see is a pop up window that says - Instructions...select items from drop down list, click outside drop down list to exit, click check boxes, etc., and then once they read the instructions and close the pop up the form opens and is ready to use.
