Not sure if it's what's causing the problem but the formulas in column B/C on the Summary Sheet only have range references down to row 14 on the Data Entry sheet.

Thanks for the reply. I already corrected it but now every time I entered new name the data itself doubled in summary sheet. Here's the link.
