Pivot table issue

I am trying to make a PivotTable or some sort of Vlookup array. so I can see who is turning in paperwork, and narrow my scope by month.

for some reason, the pivot table is not recording any of the values, and I have no clue why. The employee field seems to be fine, but the date field has weird formatting problems.
My main goal is to make it easy to print and view for each month.

It’s a very tricky solution but I think it works.

Select cells B3:HH37
Press Alt + D + P
Opens a dialog box
Select "Multiple consolidation ranges" and then click Next
Then select "I will create the page fields" and click Next
In the range field click and drag from cell B3 until HH37 and then click Finish
It will create a pivot table with your data
Right Click on cell B4 (Date 1/6/2016) ====> Group.. and choose moths
