Pivot table: issue with date column

Hello,

I'm trying to reformat the table on the left in Figure 1 to look like the table on the right in Figure 1. I thought this would be easy to do with Pivot tables, but the date is not formatting how I want it. Figure 2 shows two incorrect Pivot tables (with the settings for the Pivot table at the bottom of Figure 2).

How can I use a Pivot table to get something that looks like the table on the right of Figure 1? The Pivot table on the bottom of Figure 2 is close, but I only want one column for the dates.

I am new to Pivot tables and would greatly appreciate the help. Thank you!

Excel Facts

Spell Check in Excel
Press F7 to start spell check in Excel. Be careful, by default, Excel does not check Capitalized Werds (whoops)
Would it be possible to post that data via XL2BB instead of a picture so that I don't have to type in the data? Post data unfiltered in the original form please.

Your first pivot table is correct, you just need to click on the Design tab in pivot table tools and change the report layout to Tabular:
