One form or Mutiple Forms

Hi All!

I am currently building a database for call stats.

It will report on cash, calls, payment plans and sickness.

Is it best to make four seperate forms or combine all these into one form?

I dont want the database to be slow loading!

Is there a shortcut key for strikethrough?
Ctrl+S is used for Save. Ctrl+5 is used for Strikethrough. Why Ctrl+5? When you use hashmarks to count |||| is 4, strike through to mean 5.
One form if everything is the same except for which of the 4 things you will be "reporting" on. The form, or whatever calls the form can allow for selecting which of these four items will be shown. Opening one form only takes 1/4 of the time of opening 4 forms.
