Obtain a summary table of work incident reports

Hi guys.

I'll tell you.

I have a spreadsheet where I write down the work incident reports.

My goal is to get a table where the incident reports and justifications appear by year.

When I submit an incident report, I can do it for individual days; or for full days and indicate the ones I'm going to take.

With the justifications , there are incident reports that don't require submitting a justifications , such as vacations, but for medical appointments, for example, it does.

So, I'll show you a screenshot of the table, and what I want to get.
