Here's the deal

I have a database that tracks customers preferences of candle scents, 155 scents to be exact. I want a form that list all of the scents and each customers individual preferences. Each customer is in a Customer table, the scents are a lookup field and the preferences are in a lookup field. I want to be able to do a report and use this report to print orderforms showing individual preferences of each customer.

Thus, I need a form that once I state the CustomerID that is referenced to the Customertbl, I automatically have every scent in my tbllookupscents and the options table of the tbllookuppreferences. It would like like this

Customer ID Scent Preference
Dave Apple Spice Like
Dave Banana Nut Dislike
Dave Clove Hate
etc etc etc.

What I dont' want to do is have to choose the customer id and scent each time. So, when I click on a new entry, the Scent field is filled, the preferences are filled. All I need to do is tell it what CustomerID I am refering and then simply rate their preferences.
