Nested "if" formulars - I'm not sure if i need to use something else?

What I'm looking for is detailed in the pic below.

At the min i have this, so i have my vlookups, but it brings back "no notes" if for all cells unless they have a note. I need it to look at the entry too. Hopefully that makes sense?

Also, I find it hard to understand with your current sample data. No one knows what the Notes data looks like that your expecting to return.
Trying using simple names instead of stuff like this: asjhfkashf
It makes it difficult to reference in any followup posts.
