Need Help For Bonus Scheme

Hello,

i need help from you guys to build a form for calculating bonus.

attached is an excel and the required is described below:

1- the target is to get a bonus for a person based on the activity
2- each cell under Bonus Column must equals quantity X value for the selected task
3- the value of each tasks depends on selected person.
4- finally, when selecting person 1 and task 2, automatically selecting corrsponding value and multiply with QTY. and list under bonus column.
