*Need Help Fast with Creating an Excel Graph

Good Evening,
I'm in a bind and need to create a graph in excel tonight and I'm a little lost. I can create simple graphs no problem, but have no clue how to graph the below. Basically, I'm needing to graph patient depression levels throughout their admission to discharge from our facility. There may be up to 20 or so patients and there are 8 different levels of depression that are selected from a dropdown list. Each patients depression level may be checked from 1 to 6 times before they leave the facility. I have a log that will be completed with patient names in column A, Check #1 in column B, check #2 in column C, check #3 in column D, check #4 in column E, Check #5 in column E and check #6 in column F. columns thru F have the dropdowns with the 8 different levels that can be selected. I was told that a scatter plot might be the best way to graph this?....I have no clue how to graph this. PLEASE HELP ME!....Thanks in advance :)
 

Excel Facts

Copy a format multiple times
Select a formatted range. Double-click the Format Painter (left side of Home tab). You can paste formatting multiple times. Esc to stop
for a fast chart, select a cell in the data then press F11

for a better solution, please post better information

Hi Fazza,
Sorry if I didn't explain it the best. Basically I'm needing to graph patient levels of depression over time. "Column A" lists the patient names. "Column B" through "Column G" will have one of four entries (Minimal, Mild, Moderate, Severe). The goal is to create some sort of graph that will track each patients level of depression (graph A1:G1, graph A2:G2, etc).
