Moving items to a new page automatically

I'm trying to find a way to automatically populate the Category/Rank per site as unique rows on the Vendor – Upload Template Tab by us filling in the Vendor – Account Template Tab. Site information on the Vendor – Account Template Tab would be populated manually along with the Categories (upon learning what they are). I included some sample data on the Vendor – Account Template to simulate how things would be entered. I also manually populated the Vendor – Upload Template with what I would want the result to look like. I've tried to do this in formulas that just got way to convoluted. I was trying to do it without VBA. but I'm not sure it's possible. I'm not overly sure this can be done at all.

Actually, most of it does. The only goof I made was that I listed the one as Other Mechanical Services on the vendor upload template and Other mechanical on the vendor account template.
