Match Forumla

Good morning everyone ,

I'm trying to write a match forumla but keep getting stuck and was wondering if anyone here could help.

I want to check 3 columns (1 - 3) and if they match then return the value in the 4th column;

Col1, Name = Barbara
Col2, Sheet = Shoes
Col3, Code = 123
Col4, Data = Nike

So using the example above if Col1=Barbara, Col2=Shoes and Col3=123 then my cell value would be Nike.
