Match and Index From a Varying List of Lookup Values

Hi guys

I'm trying to find a formula that'll do three things:

1. Allow multiple criteria for lookup
2. Display multiple results from the lookup
3. Allow me to use a list of varying criteria for the lookup.

I have found formulas that allow me to do 1 and 2, but not the 3rd.

Any help would be much appreciated. I just can't figure this out.
