Macros to generate Report in MS Word from data in MS Excel

I require a macro which generate report in MS Word from the data available in MS Excel.<?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:office:office" /><o:p></o:p>
Sample report in MS Word format seeks data like “Name, Father Name, Date of Birth” etc. These data are available in excel worksheet.<o:p></o:p>
The output must be generated for each entry in excel worksheet as a separate and individual report in MS Word.<o:p></o:p>
I am using MS office2010 and my OS is Windows 7.<o:p></o:p>
Kindly help.<o:p></o:p>
 
In the word report under serial number 1, it is Name:-
under serial number 2, it is Father name:-
under serial number 3, it is Date of birth:-

The data like Name, Father name and DOB are in excel worksheet.

while running the macro, it should create individual report in MS word format from each name in excel worksheet.

The word report created is not a letter or contact details... its the report of an individual, which contains multiple details.
