Macro for Copying Data and Transposing from column to row

I'm working on an excel form to capture data that will ultimately be stored in a seperate worksheet. I have two objectives

1. Copy the data entered in a form into a seperate raw data worksheet as a rows
2. Clear the form to prepare it for a new entry that will be saved as a new line on the raw data worksheet

Quick note: I have the following ranges with fillable data on my form
  • B4:F4 (Row)
  • E8:E12 (Column)
  • C8:C12 (Column)
  • D25:D28 (Column)

Can anyone help me with this?
