Logo Getting Squashed

For some reason, my logo is getting slightly squashed when it is printed. It looks correct on the screen.

I've got the logo in the header on the top right section, the "Scale with Document" box is unchecked, "Lock Aspect Ratio" box is checked, and "Relative to original picture size" is also checked.

Is there another option somewhere that I am missing?
