Keeping Records of Values in Different Sheet

I have two sheets, sheet1 and sheet2. On Sheet1, Column A have the project names and columns K, L and U have dates that are updated on a weekly basis and just consist of dates. K column dates are entered manually, L column have conditional formula basically adds x amount of days depending on the project type and Column U is basically whatever date L gets, it is +12 days.

So here is my question, any time dates change on sheet1 on columns K, L and U, I would like these to be recorded in sheet 2 in columns E,F and H. So basically copy of the same row in sheet1 however, while site name stays the same, dates are historically kept for record purposes in one cell such as,

03/01/2018

02/01/2018

01/11/2018

And bottom dates to have strikethrough.

Any ideas on this one?
