Insert value in the excel tables using user form

Hello Everyone, i am stuck with one challenge. If anyone can help.

I am trying to input the values (name: only one column) in the table using the user form. (Userform contains a text box, 3 radio buttons and a submit button)

I have three tables in the same worksheet (example : Team A, Team B , Team C). If the user enters the name and select one of the radio button the name will be entered in the respective table. (The table contains the same same team for the entire week)

Please find the screen shot of the form and the layout of the table. (
