Insert inputs into string

Hi -- I've been trying to do this for the better part of a morning and can't get it quite right.

I'm simply trying to create a string ("output" below) that inserts inputs from various "input" cells. I know this is easy, but I'm not getting it complete.

Any ideas on how to create the cell references? The actual inputs have a variety of characters and numbers.

Thank you for any ideas or suggestions.

Hi, Jonmo1 -- thank you so much. I hadn't been using quotes around the entire formula, and I also didn't have ampersands in the right place. It probably would have taken me a while to figure this out. Thank you so much for offering your time and help. I truly appreciate it.
